Nun sag, wer ist wirksam - die Behandlung, der Behandler, oder gar der Patient? Menschen, die an einer generalisierten Angststörung leiden, können ihre Sorgen kaum kontrollieren. Die Betroffenen fühlen sich ihren Befürchtungen ausgeliefert und ihre Gedanken drehen sich wie ein Karussell mehrere Stunden am Tag um mögliche Gefahren und Bedrohungen. Patienten mit generalisierter Angststörung werden in der Routineversorgung oftmals fehldiagnostiziert und erhalten deshalb ein suboptimales Behandlungsangebot.
Lay summary

Projektziele. Das Hauptziel liegt im Verständnis der längerfristigen Wirksamkeit evidenzbasierter Humaninterventionen am Beispiel der generalisierten Angststörung. Professionelle Psychotherapien sind im Durchschnitt äußerst wirksame Behandlungen gegen generalisierte Angststörung. Nichtdestotrotz brechen zwischen 10-20 Prozent der Patienten ihre Therapien ab, erleben sich einzelne Therapeuten als notorisch unwirksam oder umgekehrt Behandlungen die die Regeln der Kunst leicht verletzen erweisen sich trotzdem als erfolgreich. 

Basierend auf Erfahrungen des Antragstellers mit Metaanalysen zur Wirksamkeit von Psychotherapie und deren Komponenten, soll primär ein neuentwickeltes Forschungsdesign getestet werden, das es ermöglicht, sowohl den Einfluss der Therapeutenvariablen (bzw. Interventionen) und den Einfluss von Patientenmerkmalen zu prüfen. Dies wird im Teilprojekt "Trial" untersucht. Im Teilprojekt "Therapeut" sollen spezifische interpersonale Merkmale der Therapeuten in einem Gruppenformat erhoben und in der Therapiestudie auf ihren Einfluß hin überprüft werden. In einem dritten (als Vergleichsstudie mit Zentren in den USA und Rumanien konzipierten) Teilprojekt zu Patientenmerkmalen sollen sowohl zeitliche Struktur und Häufigkeit wie auch Inhalte der Sorgenkomponenten auf Seiten der Angstpatienten untersucht werden. Die Teilprojekte sollen es erlauben, die verschiedenen Aspekte bezogen auf Patienten und Therapeuten in einem Prozess-Ergebnis-Modell zu prüfen. 

Wissenschaftlicher Kontext. Die Beschäftigung mit generalisierter Angststörungen ist deutlich unterentwickelt. Es fehlen angemessene klinischen Studien (RCTs) zur kurz- und längerfristigen Wirksamkeit, doch vor allem zur Frage der Indikation (Moderatoren) und der Wirkmechanismen (Mediatoren) erfolgreicher Behandlungen.

Abstract

Background. The proposed SNSF-professorship has a common theme, i.e., the understanding of the enduring efficacy of psychological treatments in individuals who suffer from general anxiety disorder (GAD). There is meta-analytic evidence that bona fide psychotherapy is an effective treatment for GAD, but also for individuals that suffers from anxiety and depression. Nonetheless, in spite of the empirical evidence of the overall efficacy, there is a lack of randomized clinical trials that systematically investigate the therapists’ variability (i. e. therapist effects) within the very same treatment protocol. In evidence-based human treatments, the health professionals are usually not blinded, because they are aware of the specific treatment that they are applying (e.g. the professionals apply a specified surgery, educational program or psychological intervention). Such conditions are common in human treatments of evidence-based medicine, nursing, social work, education, and psychological interventions more generally. The development of study designs to investigate potential health professional effects along treatment effects is a general claim for human interventions. Clinical trial development. This proposal includes one of the very first process-outcome trials in GAD that systematically examine theory-based patients’ and therapists’ characteristics simultaneously using longitudinal multilevel methodology. Along with investigating therapist and patient characteristics, two methods for timing the therapy-sessions will be compared by keeping constant the standardized treatment manual for patients with GAD: (a) prolonged focus on subtle sudden changes lasting from 7 to 20 minutes and (b) a state of the art check-in phase lasting from 5 to 10 minutes. To address the nested data structure, therapists will be allocated by a single-subject ABAB-design. A total of 80 patients (40 participants in each of the two conditions) with a primary GAD diagnosis will be treated by 20 advanced cognitive-behavioral therapists at the psychotherapy outpatient clinic in the department of psychology at the university of Zürich.
