Kurzzusammenfassung:In diesem Projekt wird eine Kombination von zwei Methoden zur Diagnose der koronaren Herzkrankheit entwickelt und validiert, die rasch und schonend (nicht-invasiv, ohne Herzkatheter) Koronarverengungen nicht nur sichtbar mach sondern auch gleich feststellt, ob diese zu einer Störung der Herzdurchblutung führen.Hintergrund:Die koronare Herzkrankheit ist die häufigste Todesursache in unserer Gesellschaft. Entsprechend werden viele Patienten mit Verdacht auf diese Krankheit abgeklärt, häufig mittels invasivem Herzkatheteruntersuch (mind. 1 Uebernachtung im Spital), wo in vielen Fällen normale Herzkranzarterien gefunden werden. Nur ca. 40% erhalte eine Kathetertherapie, von der insgesamt nur eine Minderheit profitiert bezüglich Lebensverlängerung und/oder Lebensqualität. Dies liegt daran, dass leider sehr häufig das exakte Ausmass der koronaren Herzkrankheit nicht mit dem Herzkatheteruntersuch mit genügender Sicherheit abgeschätzt werden kann. Mit nicht-invasiven Methoden - am weitesten verbreitet ist die Szintigraphie - kann das wahre Ausmass der Erkrankung, nämlich die Durchblutungsstörung erfasst werden. Neuerdings kann mit CT (Computer Tomographie) die strukturelle Erkrankung der Koronararterien, d.h. deren Verengung (Stenose), nicht-invasiv dargestellt, d.h. der Herzaktheter ersetzt werden. Eine ideale nicht-invasive Abklärung der koronaren Herzkrankheit sollte beide Aspekte, d.h. Struktur (Stenosen) und Funktion (daraus allenfalls resultierende Durchblutungsstörungen) kombinieren. Eine solche Hybrid-Untersuchung darf aber andererseits nicht zu einer zu grossen Belastung der Patienten führen. Die Strahlenbelastung von Kombinationsuntersuchungen summierten sich bisher auf max. 41mSv, und das gesammte Untersuchungsprotokoll dauerte ca. einen halben Tag.Das Ziel:1.) Das Projekt soll eine neue Untersuchungsmethode entwickeln, welche als nicht-invasive Hybrid-Untersuchung die koronare Herzerkrankung vollumfänglich abklärt2.) Die Methode soll schonend sein, d.h. in ca. einer Stunde fertig sein und im Durchschnitt eine Strahlenbelastung von 4mSv nicht überschreiten, d.h. zehn mal wemiger, als die herkömmlichen Untersuchungen.Bedeutung:Die neue nicht-invasive Methode soll künftig Koronarstenosen diagnostieren und gleichzeitig festzustellen, ob diese Stenosen eine Durchblutungsstörung verursachen, was eine optimale Wahl der Therapiestrategie (Medikmente vs Intervention)

Abstract

Clinical background: The standard method guiding decision for coronary revascularization, invasive coronary angiography (CA), is costly and associated with a considerable rate of procedural morbidity and mortality. It is disturbing that a large fraction of patients undergoing CA have normal coronaries because no accurate non-invasive pre-screening has been performed. Methodological background: Myocardial perfusion imaging (MPI) with single photon emission com¬puterized tomography (SPECT) is a well established non-invasive tool for assessing myocardial ische¬mia (no anatomic information for targeting revascularization). Recently, non-invasive computed tomo¬graphy coronary angiography (CTCA) has been introduced, not offering functional data. Hybrid SPECT/CTCA imaging would allow comprehensive information but radiation burden up to 41mSv and time consuming protocols have prevented its clinical implementation so far. RESEARCH PLAN:Phase A: Validation of MPI with an ultrafast camera with highly sensitive new generation detector systems (cadmium, zinc, and telluride, CZT) versus standard MPI-SPECT in 70 patients.Phase B: Testing the hypothesis that a rapid non-invasive hybrid (CZT-SPECT/CTCA) imaging with an ultra-short protocol time (< 1 hour) with substantially reduced total radiation dose (<4 mSv) allows evaluation of coronary lesions and their pathophysiologic relevance to reach the same conclusions with regard to treatment strategy (1. Revascularization; 2. No Revascularization, i.e. medication or no treatment) compared to the best clinical standard (invasive CA findings combined with routine SPECT-MPI or invasive fractional flow reserve measurement with pressure wire).Data acquisition: Standard of reference: Seventy to 100 patients clinically referred for elective CA for CAD assessment will undergo, first: CA to exclude or document coronary lesions and, second: evaluation of the pathophysiologic relevance of a lesion (not predictable by anatomic findings alone) by either mental integration of the previously performed routine MPI-SPECT or by use of intracoronary pressure wire. Decisions towards treatment strategies will be taken according to clinical standards.Hybrid method: Prior to the routine investigations low dose CTCA with prospective ECG triggering will be performed on a 64-slice CT scanner restricting the scan to one distinct end-diastolic phase of the RR-cycle as we have previously established, resulting in radiation doses between 1 to 3 mSv. To reduce SPECT-MPI radiation dose we introduce a stress-only protocol (avoids resting scan radiation). Only half of the standard 99mTc-tetrofosmin activity will be injected as the scans will be performed on a cardiac ultrafast camera with highly sensitive new generation detector systems (cadmium, zinc, and telluride, CZT). Furthermore, a new dedicated algorithm for iterative reconstruction will be used which allows tracer reduction by compensating for low count imaging. This camera allows shortening the acquisition time from over 15 minutes to a few minutes according to our preliminary results.Analysis: CTCA and rapid CZT SPECT-MPI will be fused to hybrid images as we have previously established and validated. Clinical conclusions reached by the hybrid image will be validated against the clinical decision reached by clinical routine standard of reference as indicated above.
