Lead: Affects disorders in mothers are common during the postpartum period just after the birth of a child, covering a spectrum from light sadness to depression. These disorders have a negative impact on the child's development. This project aims at studying the role of the father during this period.Abstract: Affect disorders in mothers are common during the postpartum period, covering a spectrum from light dysphoria or depressive mood to clinical depression, designated as a postpartum depression. These postpartum disorders have an impact on the child's development, with regard to adaptation or to psychopathological disorders. On one hand, the main variable proposed to explain this impact on the child is the disturbance of the mother-child interactions: the mother, preoccupied by her depressive affects, is not sensitive to the child's signals. On the other hand, the father-mother relationship has been described as having a major impact on the mother's depressive state, either as a factor of support (in the child psychiatry literature) or as a factor aggravating the maternal pathology (in the family therapy literature about marital conflict). As these research traditions have developed separately, an integrative approach is necessary. The aim of this research is to comprehensively study the links between the father-mother-child relationship, conceptualized as the family alliance, and the mother-child relationship and the child's development.Specific aims: this project aim at assessing triadic father-mother-child interactions and dyadic mother-child interactions in primiparous families, and their impact on the child's development over the course of his first 18 months. A longitudinal design will be used, with three points of measurement: 3, 9 and 18 months after the child's birth. The methods are: (i) assessment of interactions in semi-standardized laboratory situations by direct observation, (ii) diagnosis of maternal depression by structured interview and self -reported questionnaires, (iii) testing of the child's psychomotor development and (iv) assessment of exploratory variables by self-report questionnaires.Significance of the project: results of this research are expected to optimise the screening of at-risk families during the perinatal period, and to improve the knowledge about aggravating factors or protective factors for the child's development when the mother presents depressive features or disorders (specifically: the role of the family by inclusion of the father in the assessment). Results and knowledge coming from this study may also be used to design interventions of primary and secondary prevention.

Abstract

BackgroundAffect disorders in mothers are common during the postpartum period, covering a spectrum from light dysphoria or depressive mood to clinical depression, designated as a postpartum depression, at times reactivating depressive disorders prior to pregnancy. These postpartum disorders have an impact on the child’s development, with regard to adaptation or to psychopathological disorders. On one hand, the main mediating variable proposed to explain this impact on the child is the disturbance of the mother-child interactions: the mother, preoccupied by her depressive affects is not sensitive to the child’s signals. On the other hand, the role of the father has been described as having a major impact on the mother’s depressive state, either as a factor of support (which is described in obstetrical or child psychiatry literature) or as a factor aggravating the maternal pathology (mainly described in the literature about family therapy or marital conflict). As these two research traditions have developed separately, an integrative approach is necessary. The aim of this study is to comprehensively study the links between the mother-child relationship, family relationships and the child’s development.Working hypothesisFamily interactions are a moderating factor which attenuates or aggravates the consequences of maternal depression on the child’s development.Specific aims.To assess triadic father-mother-child interactions, dyadic mother-child interactions, and their impact on the child’s development over the course of his first 18 months.MethodFollow-up of primiparous families in a longitudinal protocol, with three points of measurement: 3, 9 and 18 months after the child’s birth. Semi-experimental design with (i) assessment of interactions in semi-standardized laboratory situations by direct observation, (ii) diagnosis of maternal depression by structured interview and self -reported questionnaires, (iii) testing of the child’s psychomotor development and (iv) assessments of exploratory variables by self-report questionnaires. The project implies collaboration between two sites to constitute the sample composed of volunteer families with mothers presenting different degrees of depressive spectrum (assessed with the DSM IV and the MADRS). The CEF (IUP, DP-CHUV, Lausanne) leads the project, carried out with the participation of the Liaison psychiatry of the maternity service of the University Hospital of Lausanne (CHUV).Expected value of the proposed projectOptimising the screening of at-risk families during the perinatal period, and improving the knowledge about aggravating factors or protective factors (specifically: the role of the family - i.e. by inclusion of the father in the assessment) for the child’s development when the mother presents depressive features or disorders. Results and knowledge coming from this study may also be used to design interventions of primary and secondary prevention, which should in turn be evaluated in a future study
