Lay summary

Early diagnosis and therapy increasingly operate at the cellular, molecular or even at the genetic level. As diagnostic techniques transition from the systems to the molecular level, the role of multi-modal imaging becomes increasingly important. The combination of positron emission tomography (PET) and magnetic resonance imaging (MRI), enabling truly simultaneous acquisition, will bridge the gap between molecular and systems diagnosis. MRI and PET offer richly complementary functionality and sensitivity; fusion into a combined system offering simultaneous acquisition will capitalise the strengths of each, providing a hybrid technology that is greatly superior to the sum of its parts. A prototype clinical whole-body dual-modality PET-MR system is installed in our department since Feb. 2010 by one of the leading commercial players in molecular imaging instrumentation for characterisation, validation, refinement and collection of PET-MR data for clinical research. We request funding to contribute to the improvement of the adopted sequential design of the hybrid PET-MR technology towards a fully integrated PET-MRI hybrid system allowing simultaneous collection of PET and MRI data as well as to develop the software tools required for accurate fusion and quantitative analysis of clinical data PET-MR sets. This work will push forward the development and use of PET-MRI - an embryonic technology that has excellent potential to become a powerful tool for preclinical research and clinical diagnosis.

The division of nuclear medicine together with national and international partners foresees to develop innovative approaches for quantitative dual-modality PET-MR imaging on the prototype whole-body PET-MR system installed in our department (Zaidi et al 2011).

Abstract

Early diagnosis and therapy increasingly operate at the cellular, molecular or even at the genetic level. As diagnostic techniques transition from the systems to the molecular level, the role of multi-modal imaging becomes increasingly important. The combination of positron emission tomography (PET) and magnetic resonance imaging (MRI), enabling truly simultaneous acquisition, will bridge the gap between molecular and systems diagnosis. MRI and PET offer richly complementary functionality and sensitivity; fusion into a combined system offering simultaneous acquisition will capitalise the strengths of each, providing a hybrid technology that is greatly superior to the sum of its parts. A prototype clinical whole-body dual-modality PET/MR system is installed in our department since Feb. 2010 by one of the leading commercial players in molecular imaging instrumentation (Philips Medical Systems) for characterisation, validation, refinement and collection of PET/MR data for clinical research. We request funding to contribute to the improvement of the adopted sequential design of the hybrid PET/MR technology towards a fully integrated PET/MRI hybrid system allowing simultaneous collection of PET and MRI data as well as to develop the software tools required for accurate fusion and quantitative analysis of clinical data PET/MR sets. This work will push forward the development and use of PET/MRI ? an embryonic technology that has excellent potential to become a powerful tool for preclinical research and clinical diagnosis. This research involves medical physicists, physicians, radiologists, radiochemists and scientists working at the Department of Radiology and Medical Informatics of Geneva University Hospital (HUG), and image processing and analysis researchers from the Swiss Federal Institute of Technology (EPFL) in collaboration with the above mentioned industrial partner who provided the prototype for this purpose.The division of nuclear medicine together with national and international partners (see attached list) foresees to develop innovative approaches for quantitative whole-body PET/MR imaging on a prototype dual-modality PET/MR system. We submit this funding request to boost our research in this area with the goals to:A.Further develop and validate PET and MRI simulation tools for accurate modelling of both subsystems, characterisation of their interactions, study of the effects of various system design parameters on imaging performance, and generation of correlated PET/MRI data sets for validation of quantitative imaging protocols of a fully integrated whole body PET/MR system (B-E);B.Develop new, more accurate, fast and robust MRI-guided attenuation compensation methods for whole-body PET imaging using optimised ultra-short TE (UTE) MRI sequences for bone segmentation combined with Atlas-guided deformable registration algorithms and neural network-based algorithm training using various anthropomorphic anatomical models;C.Develop and validate novel 4D MR-derived motion correction schemes to eliminate artefacts seen in PET-CT by developing MR-based, motion-compensated PET attenuation correction strategies:D.Develop new, more accurate, fast and robust MRI-based maximum-a-posteriori PET reconstruction (AMAP) for simultaneous partial volume correction and noise suppression in whole-body imaging using segmentation-free anatomical priors based on measures similar to mutual information;E.Develop new, more accurate, fast and robust MRI-based non-invasive arterial blood sampling for kinetic modelling by extending the use of priors into the time dimension where the temporal correlations are modelled using various basis functions such as cubic B-splines, or Markov priors.F.Validate the developed quantitative imaging methodology using simulated and experimental phantom studies and clinical data and apply it in a clinical environment in various fields including neurology (arterial spin labelling and metabolism and receptor studies), cardiology (cardiac function and perfusion), and oncology (prostate and head and neck tumours).
